The above death certificate is of my paternal 2nd-great grandfather, Eli Weldon Haley (1866-1957).  I obtained this certificate on my recent research trip into Jay County.

Facts that I already knew included the date of death and the place.  Though the certificate states the death address as simply Jay County, I know that he was living in Madison Township at the time, near Salamonia.  Interestingly, the residence address of Rural Route 3 Fort Recovery, Ohio shows how close this area is to the Ohio line.

Eli was widowed, his wife Cora Belle (Metzner) Haley having died in 1955.  His father is listed as George W. Haley and mother as Mary Jane Haley, with the notation of (Sherrick) as her maiden name.  This corresponds to facts I already knew about the family.  No places of birth are listed for his parents.

His burial date was listed as July 16, 1957 at Mt. Zion cemetery.  The funeral home was listed as the Baird funeral home of Portland.

The physician of record was S.M. Hammond, M.D.  The cause of death was not listed on this certificate.  The health department would not release the information.  Peeking over the books, I could see that the cause was heart related.
