The above death certificate is for my paternal 2nd-great grandmother, Mary Keziah (Chew) LeMaster (1847-1927). I obtained this record on my recent research trip to Jay County.
Right off the bat, the interesting fact is the spelling of the name. The surname keeps fluctuating in the records with and without a final 's'.
Facts that I already knew were her birthdate and birthplace. The certificate lists her birthplace as February 13, 1847 in New Jersey. Other sources have indicated that her birthplace was Mt. Holly in Burlington County, New Jersey.
Mary was married, and the certificate lists her husband as Laman. Whether or not this is a typographical error and a misreading of the name of her husband, Luman, I do not know as I wasn't allowed to examine the original record.
The certificate lists here father as Dr. E.C. Chew born in New Jersey and her mother as Caroline Woolston born in New Jersey. This correlates with other information I have, though her father's full name was Ezekial Cooper Chew.
Her date of burial in Salamonia, Indiana was February 25, 1927 and the funeral home was the N.R. Williamson & Son funeral home of Portland.
The physician on file was J.J. Kidder and Mary's cause of death was listed as arteriosclerosis. This agrees with her obituary, which mentions that she had suffered from heart trouble for some time. Dr. John J. Kidder was a physician in Salamonia, who began practicing there in 1896, according to the 1922 county history.
