The above death certificate is for my paternal 2nd-great grandmother, Cora Belle (Metzner) Haley (1868-1955). I obtained this record on my recent research trip into Jay County, Indiana.
Facts that I already knew included her death date and place and the date of birth. Her residence shows Portland, but she was living in rural Madison Township at the time of her death according to her obituary.
Cora was married, and her spouse was Eli Weldon Haley (1866-1957).
The death certificate lists her father as Adam Metzner and mother as Clara Moulton, which corresponds to information I already had.
The funeral was held at the Baird-Freeman funeral home in Portland. The attending physician was S.M. Hammond. The health department would not release a cause of death, but peeking over at the records I saw that carcinoma was listed.
